Chronic corporeal Achilles tendinopathies are very frequent, especially among athletes. The clinical diagnosis is generally relatively straightforward, based on the onset of spontaneous pain increased by palpation of the tendon. In more severe cases, the pain can lead to a partial or total functional impairment. The para-clinical diagnosis is confirmed by ultrasound and MRI. Initially, a medical and conservative treatment approach should be considered. If there is no improvement after at least six months, surgical treatment can be proposed. There are numerous surgical options available. In this article, the authors report their experience with longitudinal tenotomies by open surgery and demonstrate that this technique provides excellent clinical results in most cases.
