The starting point of this article is the American paradox on the issue of alcohol: the vitality of the temperance movement did not deter some segments of the medical profession from seeking early pragmatic therapeutic solutions to treat alcoholism. However, can we speak of a true medicalization of alcoholism in the sense of Irving Zola and his successors? To answer this question, the article goes back over the difficult development of an alcoholism medicine in the United States, examining whether this amounted to anything more than the proclamation that alcoholism is a disease. From the collapse of the initial alcoholism medicine in the early twentieth century until its resurgence after the Repeal of prohibition, something profoundly original finally emerges, but it’s not a conventional medicine: rather, it’s a social medicine, a medicine of the restoration of free will in a specifically American paradigm.
