Contemporary clinical analysis of tinnitus is strongly influenced by audiometry and epidemiology. Due to the modifications of this low-decibel ringing in the ear and the minority of subjects in severe distress, a behavioral habituation model has become prevalent, which does not acknowledge the extent of subjective distress. The urgency with which some patients seek help requires clinicians to adopt a rigorous epistemology of this symptom, based on psycho-acoustic studies which indicate its dynamic and variable nature. A rough clinical analysis of transference under the influence of tinnitus is proposed, keeping in mind that no one can offer the subject the aural relief that they seek so eagerly.
