Doctor-patient communication and shared medical decision-making are essential in oncology. However, contrasts between doctors’ and patients’ preferences can complicate these communication processes. This narrative review examines healthcare professionals’ experiences regarding their interactions with patients in adult and pediatric oncology settings. This paper aims to explore the ethical reflections associated with these interactions and to suggest appropriate recommendations. A literature search was carried out using PubMed, Embase, and PsycINFO, using selected keywords and focusing on relevant papers published in French or in English over the last five years. The selected studies cover various aspects regarding medical communication and shared decision-making, with a focus on clinicians’ experiences. Shared medical decision-making aims to improve patients’ autonomy and their satisfaction with care, despite barriers such as lack of time, complex information, and emotional burden, which emerged in our literature review. Managing the emotional impacts of communicating bad news represents a major challenge in oncology. Particularly challenging for physicians, communicating bad news results in significant physical and emotional reactions. Doctors must try to keep patients’ hopes up while also providing honest and appropriate information, which can be particularly challenging in pediatric oncology. This review highlights the need for improved communication and implementation of shared decision-making in oncology. Recognizing the emotional impact these situations have on physicians seems crucial in order to foster tailored and empathetic interactions with patients. Supportive strategies and improved communication training can help clinicians navigate these complex challenges, improving the quality of oncology care while caring for our doctors.
