| Summary, etc. | Introduction : L’évacuation sanitaire est l’envoi d’une personne souffrant d’un problème de santé vers une institution susceptible de lui prodiguer des soins appropriés. Il s’agit d’un processus régi par des textes juridiques. Il peut être émaillé de dysfonctionnements. Objectif : L’objectif de cet article est d’étudier le processus d’évacuation sanitaire afin de relever les éventuels dysfonctionnements. Méthodes : Une étude qualitative transversale a été menée de décembre 2017 à août 2018. La taille de l’échantillon a été déterminée par saturation. Nous avons recruté 11 personnes. Elles appartenaient toutes aux services impliqués dans le processus d’évacuation sanitaire. Les données ont été collectées à l’aide d’un dictaphone conformément à un guide d’entretien. L’analyse des données a consisté à regrouper les informations extraites du verbatim. Résultats : Pour une éventuelle évacuation, des dysfonctionnements ont été relevés dans les décisions du médecin, dans la constitution du dossier médical d’évacuation et dans la signature de l’observation clinique par les médecins. Nous avons relevé des irrégularités dans la tenue du Conseil de santé entraînant des prises de décisions arbitraires ainsi que le manque de collaboration entre les services, entravant le suivi administratif des personnes évacuées. Sur le plan financier, il apparaît que le paiement des frais d’hospitalisation et de soins médicaux était effectué au profit de comptes privés et non de ceux des hôpitaux d’accueil contrairement à ce que recommande l’arrêté d’évacuation. Conclusion : Le processus d’évacuation sanitaire en République du Congo était émaillé de dysfonctionnements dans la décision prise par le médecin, dans les décisions administratives et financières ainsi que dans la liaison avec les structures d’accueil et le suivi des malades. |

| Summary, etc. | Background: Medical evacuation is the transfer of a patient from a health institution to another for more appropriate care. The medical evacuation process is framed by legal texts. Dysfunctions are still possible. Objective: The aim of this article is to study the medical evacuation process in order to identify possible dysfunctions. Methods: A cross-sectional qualitative study was conducted from December 2017 to August 2018. We recruited 11 people according to saturation principles. They were all involved in the medical evacuation process. Data were collected according to an interview guide and recorded using a Dictaphone. Data analysis consisted of gathering the information extracted from the verbatim. Results: Dysfunctions were noted in the doctor’s decisions for a possible evacuation, the constitution of the medical evacuation file and the signature of the clinical observation by the doctors. Irregularity in the Medical Evacuation Board meetings was also noted, leading to inappropriate decision-making. We found the absence of inter-sectorial collaboration in evacuation, leading to poor follow-up of transferred patients. We also found the payment for hospitalization and medical care in private accounts instead of selected hospitals’ accounts as stated in the evacuation guide. Conclusion: The medical evacuation process in the Republic of Congo suffers from many irregularities including the doctor’s decision, administrative and financial decisions, as well as the follow-up of patient in the host country and in the hospital abroad. |
