| Summary, etc. | Les bronchopneumopathies chroniques représentent aujourd’hui un réel problème de santé publique.En grande partie dues au tabac, elles engendrent, au cours de leur évolution, un lourd handicap. A un stade avancé apparaît une insuffisance respiratoire. Les personnes ont alors besoin d’un traitement substitutif : l’oxygénothérapie de longue durée. L’étude que nous venons de mener montre qu’un tel traitement permettant l’allongement de la durée de vie, ne se justifie pas totalement en tant qu’amélioration de la qualité de vie. En effet, il semble aggraver les contraintes engendrées par la maladie elle-même. Une éducation menée sur les lieux de prescription, le développement de réseaux hôpital-prestataires à domicile et associations de malades, permettraient peut-être une meilleure acceptation et adaptation au traitement (suggestions pouvant faire l’objet de futures recherches). De même, il serait peut-être intéressant de s’interroger sur l’impact d’une sensibilisation, du grand public et des fumeurs, à une telle pathologie et au handicap qu’elle engendre. |

| Summary, etc. | Chronic bronchopneumopathies are today a real public health problem.They are mainly due to tobacco, and during their development, they generate a heavy handicap. At an advanced stage, the patient develops a respiratory insufficiency and then requires a substitution treatment : long lasting oxygen therapy. The study we have just carried out shows that such a treatment enabling to prolonge life is not totally justified as an improvement of life quality. Indeed, it seems to aggravate the constraints engendered by the disease itself. A training on the place of prescription, the development of hospital-home care service networks and patients associations, would perhaps allow a better acceptance and adaptation to the treatment (these suggestions could be the subject of future research). Likewise, it might be interesting to ponder over the impact of making the general public and the smokers sensitive to such a condition and to the handicap it generates. |
