| Summary, etc. | Cette étude qualitative porte sur vingt observations. Les problèmes de l’alimentation à la cuiller des patients hospitalisés, âgés et en perte de capacités sont rarement évoqués. Cette étude a pour but d’en analyser les particularités pratiques, conceptuelles et éthiques. Dix patients ont été l’objet d’observations directes et leurs dix infirmiers référents ont répondu à des interviews structurées. Nous avons tiré les trois constats marquants suivants :Le recours à l’alimentation à la cuiller s’est avéré inopérant parce qu’il n’a pas permis de résoudre les problèmes invoqués pour en justifier la mise en place.Trois mois après ces observations, six patients sont décédés, trois ont retrouvé leurs capacités à s’auto-alimenter et un est dans un état stationnaire.Dans tous les cas observés, l’alimentation à la cuiller a été instaurée sans processus de décision mais par une sorte de glissement tacite. Pour sortir de cette impasse, nous estimons indispensable de faire passer ce mode d’alimentation de son état actuel de devoir humain irrécusable au statut conceptuel de soin et de développer les outils d’évaluation qui permettront de l’intégrer à une stratégie structurée d’interventions infirmières. |

| Summary, etc. | This qualitative study is based on twenty observations. Problems with spoon feeding of hospitalized, elder, demented patients are rarely considered. This study aims at analyzing their practical, conceptual and ethical specificities. Ten patients were directly observed and their ten nurses of reference responded to structured interviews. We established three significant observations :the use of spoon feeding appeared inoperative as it did not help resolve the problems that were invoked to justify its implementation.three months after the observations six patients were deceased, three were able to feed themselves and one did not improve.in all observed cases, spoon feeding was initiated as a tacit evolution, with no formal decision process. To avoid this dead end we consider it indispensable to evolve this feeding practice from its current state of unchallengeable humane duty to the conceptual level of patient care, and to develop evaluation tools which will enable its integration into a structured strategy of nursing interventions. |
