| Summary, etc. | Le dossier de soins est un outil privilégié dans la pratique des soins infirmiers. Suivant les lieux, il s’est développé de façon plus ou moins importante ces trente dernières années. Dans le cadre d’une institution préparant l’informatisation du dossier patient, une évaluation a été réalisée sur le contenu de l’anamnèse et des notes quotidiennes infirmières. Cette étude portant sur 110 dossiers de soins a mis en évidence différents aspects dans la manière de transcrire tant les données factuelles que les notes issues de l’observation, de l’action et de l’évaluation. D’une part, l’impact de la démarche se traduit par des propositions concrètes auprès des équipes de soins et d’autre part à travers une réflexion exploratoire sur la traçabilité concernant les dossiers de soins. |

| Summary, etc. | Treatment records are preferential tools in nursing care. Depending on the setting, its development has gained greater importance during the last thirty years. In the environment of an organization in charge of the computerization of patient files, an evaluation was conducted on the contents of the anamneses and daily observations of nurses. This study, concerning 110 medical records demonstrated the different facets in the methods of transcribing data from notes ensuing from observations, action, and evaluation. Alternatively, the impact of this procedure translates into concrete propositions by the medical teams, as well as exploring the benefits of charting the treatment records themselves. |
