| Summary, etc. | Cet article propose une méthode interdisciplinaire de recueil et d’analyse des données de recherche de terrain en psychiatrie, combinant position ethnographique, au moyen d’observations et d’entretiens semi-dirigés, et élaborations psychodynamiques dans l’après-coup. Nous partons d’une situation d’agression d’un patient envers une soignante en unité d’hospitalisation, telle que consignée dans notre journal de terrain, qui questionne les aspects réels, symboliques et imaginaires de la liberté de circulation en psychiatrie. Nous montrons comment de telles situations complexes ne peuvent être comprises qu’avec une triple grille de lecture déclinant paramètres de l’établissement, vie institutionnelle et dynamique groupale. À l’aide de la théorie bionienne des cycles de transformations, nous faisons, enfin, une hypothèse théorique selon laquelle existeraient des transformations désymbolisantes, qui par cycles successifs contribueraient à la pathoplastie institutionnelle. Nous invitons les chercheurs en clinique institutionnelle à investiguer davantage cette hypothèse. |

| Summary, etc. | This article proposes an interdisciplinary method for gathering and analyzing field research data in psychiatry, combining an ethnographic position, by means of observations and semi-directive interviews, and retrospective psychodynamic elaborations. I take as my starting-point a situation where a patient acted aggressively towards a carer in a hospitalization unit, as recorded in my field diary, raising questions about the real, symbolic and imaginary aspects of free movement in psychiatry. I show how such complex situations can only be understood from a triple interpretative perspective that takes into account the parameters of the establishment, institutional life and group dynamics. With the help of Bion’s theory of transformation cycles, I put forward the theoretical hypothesis that de-symbolizing transformations exist which, in successive cycles, contribute to institutional pathoplasty. Researchers in institutional clinical practice are invited to investigate this hypothesis further. |

| Summary, etc. | Este artículo propone un método interdisciplinario de recogida y análisis de datos de investigación de campo en psiquiatría, combinando una posición etnográfica, mediante observaciones y entrevistas semidirigidas, y elaboraciones psicodinámicas a raíz de las mismas. Partimos de una situación de agresión de un paciente hacia una trabajadora de la salud en una unidad de hospitalización, registrada en nuestro diario de campo, que cuestiona los aspectos reales, simbólicos e imaginarios de la libertad de movimiento en psiquiatría. Mostramos cómo estas complejas situaciones sólo pueden entenderse con un triple modo de interpretación a través de los parámetros de la institución, la vida institucional y la dinámica de grupo. Utilizando la teoría bioniana de los ciclos de transformación, hacemos finalmente una hipótesis teórica según la cual existen transformaciones desimbolizantes, que por ciclos sucesivos contribuyen a la patología institucional. Invitamos a los investigadores de la clínica institucional a seguir profundizando la investigación de esta hipótesis. |
