Summary, etc. | RésuméLa méthode de stimulations électriques cérébrales a connu ses principaux développements techniques dans la seconde moitié du xxe siècle. Elle consiste, au cours d’une opération chirurgicale et alors que le patient est éveillé, à appliquer un courant électrique directement au contact du parenchyme cérébral, et à rechercher, pour la zone du langage, une réponse sous la forme d’un symptôme aphasique. Il s’agit donc de créer une lésion « virtuelle » très focalisée et d’en observer les conséquences sur le langage. Son application systématique en neurochirurgie oncologique a permis d’optimiser la chirurgie des tumeurs cérébrales, en autorisant une exérèse plus large qui respecte les fonctions cérébrales essentielles telles que le langage. L’identification de ces aires essentielles par des tâches de dénomination d’objets, de lecture ou d’écriture, a aussi permis de mieux appréhender l’organisation cérébrale des différents systèmes (sémantique, phonologique, moteur articulatoire) qui, selon différents modèles actuels, concourent à la production langagière orale ou écrite. De surcroît, certaines aires spécialisées dans le transcodage de l’information alphabétique et numérique ( e.g., aires différentes pour le traitement phonologique des mots et des chiffres arabes dans le gyrus supramarginal), voire orthographique ( e.g., buffer graphémique dans l’aire BA6) ont encore pu être mises en évidence. |

Summary, etc. | Language mapping for brain tumour resection: technical and fundamental aspectsElectrocortical stimulation mapping (ESM) is a brain mapping technique that allows examination of the brain in action. The neurosurgeon directly delivers an electric current onto the brain parenchyma with small electrodes in order to precisely locate functional areas that must be avoided during surgery. He looks for language interferences induced by stimulation whilst the patient is asked to perform linguistic tasks ( e.g., picture naming). In the field of neurosurgical oncology, refinement of neurosurgical technique and routine use of stimulation mapping has enabled safer operations with better outcomes. In the field of neuroscience, ESM studies, in which naming, reading and writing tasks have been used, have enabled a better understanding of language functional organization in the brain. The anatomical correlates of specific language-processing components (phonology, semantics, and articulatory networks) have been identified in the perisylvian region. In addition, ESM specified the dissociation of phonologic representations transcoded from either alphabetic or numeric Arabic formats of numbers in the supramarginal gyrus. ESM also revealed a frontal bridging area between orthography and motor programs specific to handwriting located in BA6. |
