| Summary, etc. | Depuis le début des années 2000, la vulnérabilité sociale est identifiée comme un risque périnatal majeur, justifiant en 2006 la création du réseau francilien Solipam (Solidarité Paris Maman Île-de-France). L’expérience des professionnels du réseau ainsi qu’une étude statistique inédite de 933 parcours de soins de familles accompagnées par Solipam permettent de mesurer le risque de santé encouru selon chaque déterminant social et de montrer qu’une coordination médicosociale personnalisée de chaque parcours de soins sur l’ensemble du territoire régional est essentielle à la bonne prise en charge de ces familles, devenues, du fait d’un environnement instable et déficient, des cas cliniques complexes. |

| Summary, etc. | Since the early 2000s, social vulnerability has been identified as a major perinatal risk factor, leading to the creation of the Paris Region network Solipam (Solidarity Paris Mother) in 2006. A statistical study of 933 healthcare circuits coordinated by Solipam measured the health risks associated with each social determinant. The results of this study show that a customized medico-social coordination of each healthcare circuit is critical to provide quality care to families living in an unstable and run-down environment, thus becoming complex clinical cases. |
