| Summary, etc. | La présente étude de cas vise à illustrer la pertinence d’un examen neuropsychologique adapté pour déterminer l’impact de décharges épileptiformes dites « infracliniques » sur le traitement de l’information. Un jeune adulte atteint d’épilepsie de type absence niant tout symptôme, mais présentant des décharges fréquentes de type pointes-ondes de courte durée (< 3 s.) sur l’EEG a fait l’objet d’une évaluation neuropsychologique comprenant une batterie de tests formels ainsi qu’une tâche de Go/No-Go réalisée simultanément à l’enregistrement EEG. L’évaluation à l’aide de tests formels n’a pas révélé de troubles cognitifs incompatibles avec la conduite automobile. Cependant, la réalisation de la tâche Go/No-Go simultanément à l’enregistrement EEG a révélé un impact significatif des pointes-ondes généralisées sur la cognition, entraînant l’omission de cibles, des mauvaises réponses et des temps de réaction plus lents – des résultats incompatibles avec une conduite automobile sécuritaire. L’introduction d’un médicament anti-crise a conduit à une régression complète des pointes et des omissions pendant la tâche. |

| Summary, etc. | This case study aims to illustrate the relevance of an adapted neuropsychological examination to determine the impact of so-called “subclinical” epileptiform discharges on information processing. A young adult patient with absence epilepsy reporting no symptoms but presenting frequent spike-and-wave discharges of short (< 3 s.) duration on the EEG underwent a neuropsychological evaluation including a battery of formal tests as well as a Go/No-Go task performed simultaneously with the EEG recording. Evaluation with formal tests did not reveal any cognitive impairment incompatible with driving. However, the performance of the Go/No-Go task simultaneously with the EEG recording revealed the significant impact of spike-and-wave discharges on cognition, resulting in target omission errors, incorrect responses, and slower reaction times—results that are incompatible with safe driving. The introduction of an antiseizure medication led to a complete regression of spikes and omissions during the task. |
