Summary, etc. | RésuméL’encéphalomyélite myalgique/syndrome de fatigue chronique (EM/SFC) est une affection intégrant notamment une fatigue persistante durant plus de 6 mois, des malaises post-effort et des troubles cognitifs caractéristiques. Si des anomalies caractéristiques ont été mises en évidence en imagerie fonctionnelle, il n’y a pas encore de mécanisme pathogène identifié, renvoyant l’EM/SFC au statut de syndrome et non de pathologie avec une étiologie déterminée. Le diagnostic repose donc sur des critères purement cliniques en l’absence de cause identifiée. Ces éléments ont pour conséquence de complexifier la phase diagnostique et d’augmenter les risques d’errance pour les patients. De fait, il apparaît intéressant de se pencher sur les éléments cognitifs inhérents à l’EM/SFC et d’utiliser les outils d’évaluation standardisés de la neuropsychologie pour dégager des éléments quantifiés à ajouter aux critères. Dans cette synthèse, nous détaillons le profil des patients et proposons une méthode d’examen cognitif pour les cliniciens de la neuropsychologie. |

Summary, etc. | Myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) is a condition involving notably persistent fatigue lasting more than six months, post-exertional malaise, and characteristic cognitive impairment. While suggestive patterns of abnormalities have been shown on functional brain imaging, no pathogenic mechanisms have yet been identified, making ME/CFS a syndrome rather than a pathology with a specific aetiology. In the absence of an identified cause, the diagnosis is based on criteria which are essentially clinical. These elements make the diagnostic phase more complex and increase the risk of an odyssey for patients before being recognized as suffering from ME/CFS. In this context, it seems appropriate to examine cognitive elements inherent to ME/CFS and to use standardized neuropsychological assessment tools to identify quantified elements to add to the existing criteria. In this paper, we propose a synthesis of ME/CFS-associated cognitive profiles and recommend a cognitive examination approach for neuropsychological clinicians. |
