| Summary, etc. | Il s’agit d’exposer notre cheminement de pensées et la naissance de nos interrogations autour de ces situations d’enfants autistes où la question d’une surveillance, voire d’une prise en charge des frères et sœurs puînés s’est posée. Nous présentons notre cas princeps, celui d’un garçon de 30 mois chez qui nous avons diagnostiqué un autisme infantile. Les éléments biographiques, l’histoire développementale et l’anamnèse des troubles ont été principalement recueillis grâce au récit de la mère. Trois consultations pédopsychiatriques et deux séances de psychomotricité ont permis l’élaboration du diagnostic. Durant le temps d’observation des troubles de ce jeune garçon, sa petite sœur de 4 mois a progressivement présenté des signes de souffrance qui nous ont paru inquiétants. La question de la prise en charge de celle-ci s’est donc posée. Nous soulevons la problématique du retentissement de l’autisme sur les fratries puînées à travers une étude des risques génétique, psychiatrique et psychologique. Le repérage précoce des signes prémorbides chez ces bébés considérés comme « à risque autistique » est important à connaître dans un but de prévention. Nous décrivons le dispositif de crèche familiale thérapeutique, développé au sein de la consultation pédopsychiatrique des 0-3 ans de l’hôpital d’Aulnay-sous-Bois, afin que les frères et sœurs d’enfants autistes aient des temps de soins précoces adaptés. |

| Summary, etc. | We present our sequence of thoughts and questions regarding the care of younger siblings of autistic children. Multiple questions are still to be answered regarding the developmental outcomes of younger siblings of autistic children. The case presented below illustrates some of the issues. Our proband case is a 30-month old male who presented signs of infantile autism. His mother described the family history, the psychomotor development and the history of the current symptoms. We were able to provide a precise and detailed diagnostic after three pediatric-psychiatric consultations and two sessions of psychomotricity. During this child’s observation period, we noticed that his 4-month old sister progressively developed concerning signs of distress. The appropriate care for this child was discussed within the team and with the family. We propose to assess the impact of autism on younger siblings by conducting a study of the genetic, psychiatric and psychologic risk factors for autism. Early diagnostic of pre-autistic symptoms is warranted in these high risk babies who would benefit from a targeted prevention program. We describe the family-oriented therapeutic day-care organized within the pediatric-psychiatric clinic for 0-3 years old in the Aulnay-sous-Bois hospital, which goal is to provide siblings of autistic children early care focusing on prevention. |
