| Summary, etc. | The busy gynecologist still reluctantly shuns away from treating patients’ sexual health concerns due to fear of “opening risky Pandora’s boxes” while lacking adequate answers. An innovative instrument – the one-page Treatment Plan Mapping Tool (TPMT) for women’s sexual dysfunctions – and the division of female sexual dysfunctions into three treatment categories, is offered to alleviate these concerns. Category (1) – the largest group combination – that includes midlife patients with sexual desire dysfunction, sexual pain at vaginal penetration, and structural discomfort with sexual functional dysfunction, is presently readily treatable by the gynecological office. For Category (2), which includes arousal and orgasmic dysfunction, gynecologists may acquire additional sex therapy skills. All Category (3), composed of Categories (1) and (2) patients presenting disturbed couple relationships, need specialized referral. A peer-testable treatment idea, namely vaginal swishing with anticonvulsants or topical antidepressants for peripheral genital neuropathies, is presented here for the first time; if proven valid, it should add to the continuous advances in sexual pharmacotherapy. Our new tool for mapping a treatment plan for women with sexual dysfunctions, by listing most advances in sexual medicine, should integrate well into the 15-minute slot-per-patient gynecological office, providing gynecologists with valid, evidence-based, and timely guidance for immediate practice use. |

| Summary, etc. | Le gynécologue hésite encore à traiter les problèmes de santé sexuelle de ses patientes, par crainte d’ouvrir une « boîte de Pandore » sans pouvoir apporter de réponses adéquates à ce qui en sortira. Nous proposons ici un outil de cartographie des troubles et des soins en vue de prendre en charge ces dysfonctions sexuelles féminines, passant par une division de ces troubles – et de leurs traitements éventuels – en trois catégories, susceptible d’atténuer ces inquiétudes. La catégorie 1 rassemble les patientes d’âge moyen souffrant de troubles du désir sexuel, de douleurs sexuelles lors de la pénétration vaginale ou d’un inconfort structurel lié à des troubles fonctionnels sexuels. Ces désordres sont traitables dans le cadre d’une prise en charge gynécologique. La catégorie 2 regroupe les troubles de l’excitation et de l’orgasme, pour lesquels les gynécologues peuvent acquérir des compétences en sexothérapie. La catégorie 3 est constituée de patientes des deux premières catégories présentant des relations de couple perturbées. Ces cas nécessitent une orientation spécialisée. Un traitement envisageable, par bain vaginal d’anticonvulsivants ou d’antidépresseurs topiques, destiné aux neuropathies génitales périphériques, est présenté ici pour la première fois. Cette approche doit encore être validée. L’outil de planification que nous présentons ici a été conçu pour être utilisé dans le cadre d’une consultation gynécologique standard, en fournissant au praticien des conseils valides, fondés sur des preuves et opportuns, pour une utilisation immédiate dans la pratique. |
