| Summary, etc. | This article aims to explore the ethical issues surrounding harm reduction (HR) within contemporary healthcare practices, drawing on the philosophical concepts of autonomy and liberty as developed by Kant and Mill. The growing influence of neoliberal logics in healthcare institutions tends to undermine the most humanistic approaches. The article therefore examines how these philosophical concepts can be mobilised to uncover these drifts and preserve the original spirit of HR, in order to prevent it from becoming a tool of control, blame, and, ultimately, marginalisation. Reflections are offered on how to reconcile respect for the autonomy of users with the protection of the most vulnerable, while reaffirming HR as a lever for self-determination. |

| Summary, etc. | Cet article propose d’explorer les enjeux éthiques de la réduction des risques (RdR) dans le cadre des pratiques de soin contemporaines, en s’appuyant sur les concepts philosophiques d’autonomie et de liberté développés par Kant et Mill. L’influence croissante des logiques néolibérales dans les institutions de soin tend à éroder les approches les plus humanistes. L’article interroge donc comment ces concepts philosophiques peuvent être mobilisés pour mettre en lumière ces dérives et préserver l’esprit originel de la RdR, afin d’éviter qu’elle ne devienne un outil de contrôle, de culpabilisation et, in fine, de marginalisation. Des pistes de réflexion sont proposées pour concilier le respect de l’autonomie des usagers et la protection des plus vulnérables, tout en réaffirmant la RdR comme un levier d’émancipation. |
