Summary, etc. | Une évaluation optimale des répercussions neurologiques des traumatismes crânio-cérébraux de l’adulte est subordonnée à une meilleure compréhension de leur physiopathologie. Après avoir rappelé les principes généraux d’objectivation d’une atteinte du système nerveux central, fonctionnelle ou lésionnelle, cet article explique le lien entre la biomécanique des traumatismes crâniens et leurs conséquences en termes de dysfonctionnement nerveux transitoire ou définitif. Cette approche physiopathologique permettra de mieux comprendre les mécanismes générateurs de plusieurs problématiques liées aux traumatismes crânio-cérébraux (commotion cérébrale, lésions axonales diffuses, lésions basifrontales, épilepsie post-traumatique) ainsi que leurs implications sur le plan expertal. |

Summary, etc. | An optimal assessment of neurological sequels of head trauma depends on a better understanding of their pathophysiology. We shall first remind some general principle of objectivation of central nervous system impairment of functional of structural origin. In a second step we shall examine the link between the biomechanics of head trauma and transient or long-lasting neurological dysfunction. This pathophysiological approach will allow to better understand the mechanisms of some problematics, which are classically associated with head trauma: brain concussion, diffuse axonal lesions, lesions of the basifrontal cortex, post-traumatic epilepsy. The implications of such understanding in terms of medical expertise will be finally dealt with. |
