| Summary, etc. | Peptic ulcer disease (PUD) is a common condition worldwide with a significant morbidity. Mortality is particularly related to complications, especially bleeding, of the disease and the patient’s comorbidities. MUGD is a multifactorial disease linked, among other things, to Helicobacter pylori (HP) infection, which plays a fundamental role in the development of peptic ulcer disease and whose eradication leads to better ulcer healing and fewer recurrences of the disease. [3] Smoking is a major independent risk factor for the development of peptic ulcer disease (PUD) and also for the occurrence of stenotic, bleeding and perforative complications. It is estimated that 60-65% of complicated PUDs occur in patients who smoke. Smoking is also associated with delayed healing of UGD, a risk of ulcer recurrence and also failure of HP eradication even after controlling for socio-demographic factors considered as main confounders [4]. These data underline the importance of treating tobacco dependence in ulcer patients, as well as the importance of knowing the interaction between tobacco and HP. Hence the interest of this update on this pathology in relation to the effect of tobacco and HP to improve the management of MUGD diagnostic and therapeutic integrated with multiple gastroenterological, microbiological and molecular as well as behavioural mechanisms. |

| Summary, etc. | La maladie ulcéreuse gastro-duodénale (MUGD) est une affection fréquente dans le monde avec une morbimortalité non négligeable. La mortalité est particulièrement liée aux complications, surtout hémorragiques, de la maladie et des comorbidités du patient. La MUGD est une pathologie multifactorielle liée, entre autres, à l’infection à Helicobacter pylori (HP) qui joue un rôle fondamental dans le développement de l’ulcère gastro-duodénal et dont l’éradication permet une meilleure cicatrisation ulcéreuse et moins de récidives de la maladie. [3] Le tabac est un facteur de risque majeur indépendant de développement de l’ulcère gastro-duodénal (UGD) mais également de survenue des complications sténosantes, hémorragiques et perforatives. On estime que 60 à 65 % des UGD compliqués surviennent chez des patients fumeurs. Le tabac serait également associé au retard de cicatrisation de l’UGD, à un risque de récidive ulcéreuse et aussi d’échec d’éradication d’HP même après contrôle des facteurs sociodémographiques considéré comme principaux confondants [4]. Ces données soulignent l’importance du traitement de la dépendance au tabac chez les patients fumeurs ulcéreux, ainsi que l’importance de connaissance de l’interaction entre tabac et HP. D’où l’intérêt de faire cette mise au point sur cette pathologie en relation avec l’effet du tabac et d’HP pour améliorer la prise en charge de la MUGD diagnostique et thérapeutique intégrée aux mécanismes multiples gastro-entérologie, microbiologique et moléculaire ainsi que comportementale. |
