| Summary, etc. | Blastomycosis, or Gilchrist’s disease, is a chronic granulomatous infection caused by a dimorphic fungus: Blastomyces dermatitidis. Endemic in North America and sporadic in Africa, rare cases have been described in Morocco. We report a case of cutaneous blastomycosis diagnosed in the parasitology-mycology laboratory of the Hassan II University Hospital in Fez. Case report: An 88-year-old diabetic patient undergoing treatment presented with warty lesions on the face and lower limbs with no associated signs. Mycological examination of a skin biopsy confirmed the presence of B. dermatitidis on direct examination in the fresh state. An extension work-up to look for visceral involvement was negative. The patient was put on antifungal treatment with good clinical improvement. Discussion and conclusion: Blastomycosis is a rare systemic fungal infection in Morocco. The portal of entry is often pulmonary; isolated cutaneous involvement is exceptional. Diagnosis is essentially anatomopathological, and mycological studies are useful in confirming the diagnosis. |

| Summary, etc. | La blastomycose, ou maladie de Gilchrist, est une infection granulomateuse chronique due à un champignon dimorphique: Blastomyces dermatitidis, endémique en Amérique du Nord et sporadique en Afrique. De rares cas ont été décrits au Maroc. Nous rapportons un cas de blastomycose cutanée diagnostiqué au laboratoire de parasitologie-mycologie du CHU Hassan II de Fès. Observation : il s’agit d’une patiente âgée de 88 ans, diabétique sous traitement qui a présenté des lésions verruqueuses siégeant au niveau du visage et des membres inférieurs sans signes associés. Un examen mycologique réalisé sur biopsie cutanée a confirmé la présence de B. dermatitidis à l’examen direct à l’état frais. Un bilan d’extension à la recherche d’une atteinte viscérale était revenu négatif. La patiente a été mise sous traitement antifongique avec une bonne amélioration clinique. Discussion et conclusion : la blastomycose est une infection fongique systémique rare au Maroc. La porte d’entrée est souvent pulmonaire, l’atteinte cutanée isolée est exceptionnelle. Son diagnostic est essentiellement anatomopathologique ; l’étude mycologique revêt un intérêt dans la confirmation diagnostique. |
