| Summary, etc. | Tabetic arthropathy is a destructive neurogenis affection, currently rare thanks to the early treatment of syphilis with penicillin G. The germ in question is Treponema pallidum. The diagnosis is evoked in the face of the discrepancy between the importance of joint damage and the absence of pain and confirmed by the positivation of syphilitic serologies.Observation58-year-old patient, with a history of a syphilitic chancre twice, consulting for chronic pain in the left knee with arthritis. Knee radiography showed destruction of the internal tibial acetabulum and CT scan of the same limb revealed lesions of osteolysis of the two tibial plates and tibial spines with detachment of bone fragments. The diagnosis was confirmed by the positivation of syphilitic serologies in the blood and the CSF. The evolution was marked by the patient’s death from sepsis following a nosocomial infection.ConclusionEven if it has become exceptional, tabetic arthropathy should not be overlooked.Its treatment is based on penicillin G intravenously. |

| Summary, etc. | L’arthropathie tabétique est une affection neurogène destructrice, devenue rare grâce au traitement précoce de la syphilis par la pénicilline G. Le germe en cause est Treponema pallidum. Le diagnostic est évoqué devant la discordance entre l’importance de l’atteinte articulaire et l’absence de douleur. La confirmation est biologique, par la positivation des sérologies syphilitiques.Observationun patient de 58 ans, ayant comme antécédents deux occurrences de chancre syphilitique, consulte pour une monoarthrite chronique du genou gauche sans douleur. La radiographie du genou montre une destruction du cotyle tibial interne et le scanner met en évidence des lésions d’ostéolyse des deux plateaux tibiaux et des épines tibiales avec détachement de fragments osseux. Le diagnostic est confirmé par la positivation des sérologies syphilitiques dans le sang et le liquide céphalo-rachidien. L’évolution était marquée par le décès du patient par un sepsis faisant suite à une infection nosocomiale.Conclusionmême si elle est devenue exceptionnelle, l’arthropathie tabétique ne doit pas être méconnue. Son traitement repose sur la pénicilline G en intraveineuse. |
