Summary, etc. | Hyperferritinaemia is a frequent anomaly, which is admittedly not very specific, but is also of great diagnostic value if the clinician is able to observe a certain number of interpretation rules. Physiologically, ferritin is a protein synthesised mainly by the liver and designed to store iron in the liver, macrophages and erythrocytes. Despite this, pathophysiology tells us that the increase in ferritinemia goes far beyond iron overload to encompass other factors, in particular inflammation, cell lysis and distribution abnormalities. In the vast majority of cases, the cause of hyperferritinaemia can be identified using a global approach that incorporates the ferritinaemia value, the history and the patient’s clinical and biological context, without having to go through the transferrin saturation coefficient and in-depth liver investigations. Recourse to CST, MRI, genetic tests or even liver biopsy is only necessary in rare situations where the most obvious causes cannot be identified. Finally, recent years have seen the arrival of other entities, in particular SARS-COV2 infection, where ferritin has proved to be of great value both diagnostically and prognostically. |

Summary, etc. | L’hyperferritinémie est une anomalie fréquente, certes très peu spécifique, mais qui peut être d’une grande valeur diagnostique si le clinicien arrive à respecter un certain nombre de règles d’interprétation. Physiologiquement, la ferritine est une protéine synthétisée essentiellement par le foie et destinée à mettre en réserve le fer au niveau hépatique, macrophagique et érythrocytaire. Malgré cela, la physiopathologie nous apprend que l’augmentation de la ferritinémie dépasse de très loin le cadre de la surcharge en fer, pour englober d’autres volets, notamment l’inflammation, la lyse cellulaire et les anomalies de répartition. Une approche globale intégrant la valeur de la ferritinémie, les données de l’anamnèse et le contexte clinicobiologique du patient, permet, dans la grande majorité des cas, de trouver la cause de l’hyperferritinémie sans passer par le coefficient de saturation de la transferrine (CST) et les explorations hépatiques approfondies. Le recours au CST, à l’IRM, aux tests génétiques voire à la biopsie hépatique ne s’avère en effet nécessaire que dans les situations, rares, où les causes les plus évidentes n’ont pu être identifiées. À noter enfin que les dernières années ont connu l’arrivée d’autres entités, en particulier l’infection par le Sars-Cov-2, dans lesquelles la ferritine s’est avérée d’une grande valeur à la fois diagnostique et pronostique. |
