| Summary, etc. | La médecine de la personne est avant tout une médecine de la complexité de la personne qui lutte activement contre l’idée que la maladie soit l’unique objet du diagnostic médical. Ce risque est présent dans toutes les branches de la médecine ; mais il l’est avec une particulière acuité en psychiatrie où la volonté de faire science malgré tout, se paye du rejet de tout ce qui relève du subjectif ou du contexte, alors même que tout laisse à penser que ce sont là des dimensions essentielles de la personne qu’il s’agit de prendre en compte (I am I and my circumstances). L’une des premières tâches que s’est fixée la psychiatrie de la personne a été de créer un outil diagnostic permettant de ne plus devoir renoncer à prendre en compte la subjectivité pour diagnostiquer et classer : c’est le Diagnostic Intégré Centré sur la Personne (Person-centered Integrated Diagnostic-PID).Après avoir présenté cet instrument, cet article se demandera en quoi le Big-Data peut contribuer à l’amélioration de cet instrument en en faisant un allié de la médecine de la personne. Nous examinerons ainsi comment, alors que les données massives se situent a priori du côté de la médecine personnalisée concernant la personne définie par ses biomarqueurs (ce que X Guichet désigne comme la personne-moléculaire), elles peuvent aboutir à servir la médecine centrée sur la personne, c’est-à-dire celle qui concerne les personnes-sujet, qui transcendent suffisamment les données biologiques pour être des sujets autonomes. En conduisant à se demander s’il ne dissout pas la différence entre personne moléculaire et personne sujet, le Big data devient une question pour la psychiatrie d’aujourd’hui. |
