Summary, etc. | The creation of Soins Médicaux et de Réadaptation conduites addictives (SMR-A) (Medical Care and Rehabilitation for Addictive Behaviors) treatment centers in 2022 is part of the long history of care and support facilities for people with substance use disorders. The 2008 reform, which led to the creation of Soins de Suite et de Réadaptation addictologique (SSR-A) (Subacute and Rehabilitation Care Facilities for Addiction), gave existing care units the right to specialize in the treatment of all substance use disorders. The 2022 reform reaffirms this objective and goes further by encouraging them to diversify their rehabilitation services, while refocusing on their core health mission. Certain SMR-As will be chosen to become expert centers for the treatment of addiction-related cognitive disorders. The creation of partial hospitalization programs, which was previously optional, will now be mandatory for all SMR-As. At the same time, the reform of SMR funding, which has led to the introduction of stay-based charges, will force some establishments to revise their operating model. |

Summary, etc. | La création des Soins Médicaux et de Réadaptation conduites addictives (SMR-A) en 2022 s’inscrit dans la longue histoire des lieux de soins et d’accueil pour les troubles de l’usage. La réforme de 2008, à l’origine des Suites de Soins et de Réadaptation addictologique (SSR-A), reconnaissait aux unités de soins existantes la mention de spécialisation pour la prise en charge de l’ensemble des troubles de l’usage. La réforme de 2022 réaffirme cet objectif et va plus loin en les incitant à diversifier leur offre de réadaptation, tout en les recentrant sur leur cœur de mission sanitaire. Certains SMR-A seront identifiés pour devenir des centres experts pour la prise en charge des troubles cognitifs liés aux addictions. Optionnelle jusqu’alors, la création de secteurs d’hospitalisation à temps partiel devient une obligation pour l’ensemble des SMR-A. En parallèle, la réforme du financement des SMR, qui introduit désormais une large part de valorisation au séjour, va obliger certains établissements à réviser leur modèle de fonctionnement. |
