Summary, etc. | Introduction: The integration of digital tools in surgical planning has transformed the collaboration between orthodontists and maxillofacial surgeons, enabling optimized communication between practitioners as well as rapid and nearly unlimited rehearsal and adjustments. Materials and Methods: A virtual surgical planning protocol based on the “occlusal block” concept is described, structured into two complementary phases. The first phase, led by the orthodontist, involves defining the target occlusion using digital models. This “orthodontic occlusal block” is then transferred to the operating room via a final splint, ensuring faithful reproduction of the planned objectives. The second phase, carried out by the surgeon, consists of a three-dimensional evaluation of the occlusion within the overall facial context. This step accounts for facial harmony, functional constraints, and bone interferences. If this analysis indicates the need for bimaxillary surgery, the transfer is carried out using an intermediate splint. Results: In the three clinical cases presented, the authors observed a postoperative occlusion that accurately matched the jointly performed simulation. Orthodontic finishing was achieved quickly. Conclusion: This integrative approach, combining virtual planning and computer-assisted manufacturing, improves the accuracy of results, reduces complications, and enhances communication between practitioners. |

Summary, etc. | Introduction : L’intégration des outils numériques dans la planification chirurgicale a transformé la collaboration entre orthodontistes et chirurgiens maxillo-faciaux, ce qui permet une communication optimisée entre praticiens ainsi qu’une répétition et un ajustement rapides et quasi illimités. Matériel et méthodes : Un protocole de planification basé sur le concept de « bloc occlusal » est décrit, structuré en deux phases complémentaires. La première, pilotée par l’orthodontiste, consiste à définir l’occlusion cible à partir de moulages numériques, qui sera transférée au bloc opératoire à l’aide d’une gouttière finale, assurant une reproduction fidèle des objectifs planifiés. La seconde phase, menée par le chirurgien, consiste en l’évaluation tridimensionnelle de l’occlusion dans le contexte global du visage. Cette étape prend en compte l’harmonie faciale, les contraintes fonctionnelles et les interférences osseuses. Si cette analyse justifie une chirurgie bimaxillaire, le transfert est réalisé à l’aide d’une gouttière intermédiaire. Résultats : Dans les trois cas cliniques présentés, les auteurs ont observé une occlusion post-opératoire fidèle à la simulation conjointement réalisée. Les finitions orthodontiques ont pu être réalisées rapidement. Conclusion : Cette approche intégrative, combinant planification virtuelle et fabrication assistée par ordinateur, améliore la précision des résultats, réduit les complications et favorise une communication optimisée entre praticiens. |
