Summary, etc. The understanding of psychological trauma has significantly evolved, highlighting the distinction between Post-Traumatic Stress Disorder (ptsd) and Complex Post-Traumatic Stress Disorder (c-ptsd) or Developmental Trauma Disorder (dtd). Traditionally, ptsd is associated with a single or repeated traumatic event, leading to symptoms such as flashbacks, avoidance, and hypervigilance. These manifestations can significantly impair the daily lives of affected individuals. Complex trauma refers to the consequences of chronic situations of violence, abuse, or neglect during childhood, resulting in persistent difficulties in identity, emotional regulation, and relationships. Individuals with complex trauma exhibit significant adaptation challenges across various areas of their lives, which do not diminish merely with the passage of time. These challenges manifest in a range of disorders, including depression, attention-deficit/hyperactivity disorder (adhd), oppositional defiant disorder (odd), conduct disorder, anxiety disorders, eating disorders, sleep disorders, communication disorders, separation anxiety disorder, and reactive attachment disorder. Addressing these disorders involves a developmental extension of psychodynamic psychotherapy. Its practice and effects are facilitated by epigenetics and neural plasticity, as well as through transference and a (re)structuring process involving attention, active proximity, patient-therapist synchrony, reduction of dissociation, access to bodily memory, and the engagement of mentalization as a complement to attachment. Introducing the patient’s lived experience and approaching conscious thought enable access to the language of previously inaccessible experiences. Close collaboration among various professionals is often necessary.

Summary, etc. La compréhension des traumatismes psychiques a considérablement évolué en mettant en évidence la distinction entre le trouble du stress post-traumatique (tspt) et le traumatisme complexe (c-tspt) ou trauma développemental (ttd). Traditionnellement, le tspt est associé à un événement unique ou répété traumatisant, entraînant des symptômes tels que reviviscences, évitement et hypervigilance. Ces manifestations peuvent altérer significativement la vie quotidienne des individus affectés. Les traumatismes complexes se rapportent aux conséquences de situations chroniques de violence, d’abus ou de négligences durant l’enfance, qui engendrent des difficultés persistantes aux plans identitaire, émotionnel et relationnel. Les personnes qui présentent un traumatisme complexe manifestent des difficultés d’adaptation importantes dans différentes sphères de leur vie, qui ne s’amenuisent pas avec le simple passage du temps. Elles s’expriment dans une série de troubles tels que la dépression, le trouble déficitaire de l’attention/hyperactivité, le trouble oppositionnel avec provocation (top), le trouble des conduites, les troubles anxieux, les troubles de l’alimentation, les troubles du sommeil, les troubles de la communication, le trouble d’anxiété de la séparation et le trouble réactif de l’attachement. La prise en charge de ces troubles implique une extension développementale de la psychothérapie psychodynamique. Sa pratique et ses effets sont rendus possibles par l’épigénétique et la plasticité neuronale, mais aussi par le transfert et une (re)structuration engagée à partir de l’attention, d’une proximité active, de la synchronie patient-thérapeute, la réduction de la dissociation, l’accès à la mémorisation corporelle et l’engagement de la mentalisation comme complément de l’attachement. L’introduction du vécu du patient et l’approche de la pensée consciente rend possible l’accès au langage d’une expérience jusque-là inaccessible. Une collaboration étroite entre différents intervenants s’avère souvent nécessaire.
