| Summary, etc. | Les évolutions du système de santé imposent aux établissements de transformer profondément leur fonctionnement et ainsi aux acteurs d’innover dans leur pratiques organisationnelles et managériales. L’article présenté s’intéresse à l’introduction au sein des hôpitaux du lean healthcare, considéré comme une innovation managériale centrée sur la valeur perçue par le patient dans un objectif d’amélioration de la qualité des soins par une contribution de l’ensemble des acteurs dans la recherche de solutions dans la prise en charge [1]. Après une description du lean management, sa capacité à répondre aux difficultés organisationnelles rencontrées dans les hôpitaux est explorée. Dans ce sens, son caractère multidimensionnel est développé à partir de la grille de lecture de Hatchuel et Weil [2]. La question de recherche suivante est formulée : quelle dynamique ingéniérique de la démarche lean healthcare favorise son efficacité et son acceptabilité au sein de la complexité organisationnelle des hôpitaux ? Afin de répondre à cette problématique, nous mobilisons une recherche-intervention conduite en immersion profonde pendant huit mois au sein d’un hôpital luxembourgeois au plus près du déploiement d’une démarche lean healthcare. Les résultats décrivent de manière chronologique la dynamique ingéniérique de l’approche lean healthcare et permettent d’identifier l’influence de chacun des outils. Nous discutons les résultats en abordant l’efficacité des outils et leur acceptabilité sous le prisme de la combinaison des différentes composantes et de la portée de chacune d’elle. |

| Summary, etc. | The evolutions of the health system require establishments to radically transform their functioning and thus actors to innovate in their organizational and managerial practices. The article presented focuses on the introduction into hospitals of lean healthcare, considered as a managerial innovation centered on the value perceived by the patient with the objective of improving the quality of care through a contribution from the whole. actors in the search for solutions [1]. After a description of lean management, its ability to respond to the organizational difficulties encountered in hospitals is explored. In this sense, its multidimensional character is developed from the reading grid of Hatchuel and Weil [2]. The following research question is formulated: what engineering dynamic of the lean healthcare approach promotes its efficiency and acceptability within the organizational complexity of hospitals? In order to respond to this problem, we are mobilizing a research-intervention carried out in deep immersion for eight months in a Luxembourg hospital as close as possible to the deployment of a lean healthcare approach. The results chronologically describe the engineering dynamics of the lean healthcare approach and identify the influence of each of the tools. We discuss the results by addressing the effectiveness of the tools and their acceptability through the prism of the combination of the different components and the scope of each. |
