| Summary, etc. | Objectif : La stratification en catégories de risque, selon des facteurs génétiques et cliniques, permettra bientôt d’améliorer les programmes de dépistage du cancer du sein. Nous avons voulu comprendre l’influence des dimensions organisationnelles sur l’éventuelle implantation de cette approche au Québec. Méthodes : Des entretiens semi-dirigés ont été effectués auprès de 16 décideurs et gestionnaires du programme québécois de dépistage du cancer du sein (PQDCS). Un cadre d’analyse institutionnel a été retenu pour analyser les données. Résultats : L’analyse thématique des entretiens a permis de dégager un consensus sur la nécessité d’implanter une approche davantage personnalisée, fondée sur la stratification du risque, en complémentarité avec le PQDCS. Plusieurs interviewés se sont montrés préoccupés par les besoins en termes de ressources humaines ainsi que par le rôle que médecins et infirmières pourraient être appelés à jouer. L’adaptation des outils de communication aux caractéristiques des populations locales, l’équité interrégionale dans l’accès aux services, et les effets sur le taux de participation au programme organisé en place (PQDCS) ont aussi été soulevés par les interviewés. Conclusion : Notre analyse fait ressortir l’importance du contexte organisationnel du système de soins où s’implantera l’approche par stratification du risque. La disponibilité de ressources humaines formées adéquatement, l’adaptation des outils aux réalités sociodémographiques, et la compatibilité avec les mesures de la performance constituent des éléments-clés à considérer. |

| Summary, etc. | Influence of organizational factors on implementation of a personalized approach to breast cancer screening Objective: Stratification of individuals into risk categories according to genetic and clinical factors will soon improve breast cancer screening programs. This study assessed the influence of organizational factors on the possible implementation of this approach in Quebec. Methods: Semi-structured interviews were conducted with 16 managers and decision-makers involved in the Quebec breast cancer screening program (PQDCS). Results: Thematic analysis of the interviews identified a consensus among interviewees on the need to implement a more personalized risk stratification approach that would be complementary to the PQDCS. Several interviewees were concerned about the human resources required and the roles that doctors and nurses may be called upon to play. Interviewees also raised the issues of adaptation of communication tools to the characteristics of local populations; inter-regional equity of access to services; and the possible effects on the participation rate in the current program (PQDCS). Conclusion: This analysis highlights the importance of the organizational context of the healthcare system within which the risk stratification approach is implemented. Availability of qualified human resources, adaptation of tools to socio-demographic realities and compatibility with measures of performance are among the key considerations. |
