| Summary, etc. | Introduction : Malgré la reconnaissance du rôle des mutuelles de santé (MS) comme dispositifs permettant d’assurer la pérennité du financement des systèmes de santé, des études récentes ont montré l’existence de freins à l’enrôlement et à la fidélisation dans ces MS. L’objectif de l’étude était d’identifier les déterminants à l’adhésion et à la fidélisation dans les MS au Sénégal. Méthodologie : La collecte des données s’était déroulée de juin à septembre 2015 dans la région de Ziguinchor à partir d’un échantillon stratifié à deux degrés (n = 912 ménages). L’analyse a été effectuée à travers une régression multi logistique grâce à une sélection des prédicteurs par la méthode Stepwise Backward basée sur le rapport de vraisemblance. Résultats : La faible adhésion était significativement associée à la résidence en milieu rural ; à la grande taille du ménage ; au faible niveau d’instruction et de revenu du chef de ménage ; à une perception peu élevée de la qualité des soins et à l’éloignement de la formation sanitaire la plus proche. Par ailleurs, la faible fidélisation dans les MS était significativement associée au faible revenu des ménages, à la perception peu développée de la qualité des soins, à la présence dans le ménage d’une personne âgée de plus 60 ans. Conclusion : Pour une réussite de la stratégie de développement de la Couverture Sanitaire Universelle, la prise en compte de déterminants à l’adhésion et à la fidélisation des usagers dans les MS s’avère nécessaire. |

| Summary, etc. | Determinants of enrolment and retention in mutual health organization in the region of Ziguinchor (Senegal) Introduction: Despite the recognition of the role of Mutual Health Organization (MHO) as a mechanism to ensure the financial sustainability of health systems, recent studies have shown the existence of obstacles to enrolment and retention in these MHOs. The objective of this study was to examine the determinants of enrolment and retention in MHO in Senegal. Methods: Data collection was conducted from 22 June to 30 September 2015 in Ziguinchor, located 450 km from Dakar by stratified two-stage sampling (n = 912 households). The analysis was conducted by multiple logistic regression with selection of predictors in the Stepwise Backward method based on the likelihood ratio. Results: Low enrolment was significantly associated with rural residence, size of the household, low level of household head literacy and income, poor perception of the quality of care and distance from the nearest health facility. Low retention in MHO was also significantly associated with low household income, poor perception of quality of care, and presence of a person over the age of 60 years in the household. Conclusion: The determinants of enrolment and retention of consumers in MHO must be taken into account to ensure successful development of a Universal Health Coverage strategy. |
