| Summary, etc. | Objectif et méthode : Après un cancer, les programmes de prévention recommandent une activité physique régulière et une alimentation équilibrée. Issu d’une recherche qualitative selon une approche de philosophie du soin, cet article vise à mieux comprendre l’expérience des personnes après un programme d’éducation thérapeutique du patient (ETP). À la fin du traitement d’un cancer du sein localisé, des entretiens ont permis d’analyser l’expérience de 23 femmes âgées de 30 à 84 ans dont la moitié avait suivi un tel programme. Résultats : Les récits des femmes diffèrent selon leur participation au programme. Celles qui ne l’ont pas suivi n’ont pas de pratique spécifique d’activité physique ou d’alimentation ni de « rapport à soi » modifié par la connaissance des recommandations et du lien entre mode de vie et cancer. À l’inverse, les femmes qui ont suivi le programme connaissent ceux-ci. Deux groupes se distinguent.Le premier groupe croit en l’existence d’un lien entre mode de vie et cancer. Parmi ces femmes, un premier sous-groupe a pu changer de mode de vie facilement et présente un rapport à soi caractérisé par la « performance ». Dans l’autre sous-groupe, la pratique est difficile et le rapport à soi est marqué par la « responsabilisation » voire la culpabilité.Le second groupe ne croit pas au lien. Les pratiques suivent ou non les recommandations selon l’appétence des personnes. Le rapport à soi est ici de l’ordre de « l’expression de soi ». Conclusion : L’éducation du patient après un cancer peut induire des expériences positives et négatives, dont il serait nécessaire d’analyser les rapports avec les caractéristiques des programmes mis en place. |

| Summary, etc. | Objective and methods: Prevention programs recommend regular physical activity and a balanced diet after cancer treatment. Based on a qualitative research using a medical philosophy approach, this article aims to better understand people’s experience after an educational therapy program. At the end of treatment for localized breast cancer, interviews were conducted to analyze the experience of 23 women aged 30 to 84 years, half of whom had followed such a program. Results: Women’s narratives differed according to their participation in the program. Those who did not attend had no specific physical activity or diet. They made no link between lifestyle and cancer. Conversely, women who attented the program were aware of prevention recommendations. They acknowledged the link between lifestyle and cancer. Two groups stand out.The first group believes in this link. Among these women, a first sub-group was able to change their lifestyle easily. They developed a “performing” self-relationship. In the other subgroup, the practice is difficult and the relationship is marked by “self-responsability” and even guilt.The second group does not believe in the link. Practice may or may not follow the recommendations. It depends on people’s preferences. The self-relationship is “self-expression”. Conclusion: Patient education after cancer can lead to both positive and negative experiences. We need to analyze this in relation to the programs’ characteristics. |
