| Summary, etc. | Cet article propose une réflexion, à partir d’un dispositif de soin psychique groupal, ajustée à la problématique des sujets qui ne fréquentent pas les lieux de soin et que l’on retrouve en panne au cœur du socius. Ces sujets, généralement répertoriés sous le vocable de sujets non demandeurs de soin présentent une organisation psychique spécifique qui nécessite de conduire un travail de repérage rigoureux afin d’appréhender leur fonctionnement et de penser, en amont, à leur condition d’accès au soin. À l’aide d’une situation clinique retraçant deux ans de prise en charge plurifocale, l’auteur théorise un dispositif de soin institutionnel en appui sur une pratique d’accompagnement en milieu ouvert qui intègre une fonction phorique articulée à un appareil psychique groupal de suppléance. |

| Summary, etc. | This article offers reflections, with reference to a group psychic treatment setting, on the problem of subjects who do not frequent treatment centres and who are found malfunctioning within society at large. These subjects, generally classified as subjects who do not seek treatment, present a specific psychic organization that requires a rigorous work of identification in order to understand their way of functioning and thinking prior to granting them access to treatment. With the help of a clinical situation retracing two years of multifocal provision of care, the author theorizes an institutional treatment setting supported by a practice of accompaniment in the community which integrates a phoric function linked to a supplementary group psychical apparatus. |

| Summary, etc. | Este artículo propone una reflexión, a partir de un dispositivo de tratamiento grupal, ajustada a la problemática de sujetos que no frecuentan los lugares terapéuticos y a los que encontramos dañados en el corazón del socius. Estos sujetos, generalmente identificados como sujetos que no solicitan tratamiento presentan una organización psíquica específica que necesita llevar a cabo un trabajo de identificación riguroso para aprehender su forma de funcionar y de pensar, antes de su condición de acceso a un tratamiento. Con la ayuda de una situación clínica que traza dos años de tratamiento multifocal, la autora teoriza un dispositivo de terapia institucional en apoyo a una práctica de acompañamiento en un medio abierto que integra una función fórica articulada a un aparato psíquico grupal de suplencia. |
