| Summary, etc. | RésuméCette intervention vise à démontrer que la psychanalyse, depuis Freud et Lacan, peut permettre aux médecins généralistes une analyse plus pertinente des actes de langage et des idéologies à l’origine des symptômes. Parce qu’elle n’est « pas toute » psychothérapeutique, la psychanalyse permet, en effet, de mieux cerner les enjeux d’une politique de santé mentale qui tente d’adapter les soins aux facteurs de l’économie moderne. Nous chercherons à expliquer comment, pour faire face aux exigences des « usagers de la santé », les médecins généralistes sont poussés à ignorer les problématiques induites par la notion d’inconscient freudien, au profit d’une modélisation neurobiologique plus facilement évaluable et quantifiable. Enfin, nous démontrerons que le burn-out, version moderne de la « livre de chair » shakespearienne, est souvent le prix à payer pour le médecin lorsqu’il évacue le « réel » de l’inconscient freudien au profit d’un « symbolique » noué à un « imaginaire » plus facilement maîtrisable. |

| Summary, etc. | Medecine, psychoanalysis and psychiatryThis paper aims at demonstrating how psychoanalysis, since Freud and Lacan, has been helping general practitioners to accurate their analyses of speech acts and ideologies which instigate many symptoms. Because psychoanalysis is « not all » about psychotherapy, it helps to better understand the issues of a mental health policy which tries to adapt care to economics. We want to demonstrate how health care users demands bring GPs to ignore the freudian notion of the unconscious to the benefit of a neurobiological modelling, easier to measure. Finally we want to show that GPs burn-out, a modern version for the shakespearian « pound of flesh » is the price to pay for giving up and repressing the « real » of the Freudian unconscious, to the benefit of a « symbolic » tied with an « imaginary », much more controllable. |
