Summary, etc. La thérapie familiale a une place de choix dans le traitement de l’anorexie de l’adolescent. Mais certaines familles (environ 10%) répondent mal à cette approche, la refusent ou la trouvent insuffisante. L’efficacité reconnue de l’approche multifamiliale dans le traitement de divers troubles psychiatriques majeurs nous a encouragées à développer un programme de thérapie multifamiliale ambulatoire appliqué à certaines adolescentes anorexiques et leurs familles après une hospitalisation. L’article décrit le projet thérapeutique, ses théories sous-jacentes et ses objectifs, sa structure et ses activités, ses avantages et ses difficultés. Le groupe multifamilial constitue une modalité thérapeutique intéressante pour accompagner les cas difficiles, restant en situation de fragilité après l’hospitalisation et présentant un risque important de rechute et de chronicisation.

Summary, etc. Multifamily group treatment of adolescent anorexia nervosa.Family therapy is one of the most effective treatments of adolescent anorexia nervosa. Nevertheless, some families (approximately 10%) do not respond well to this approach, refuse it or are not entirely satisfied with it. Inspired by the multifamily group approach developed in the 60s and 70s to treat schizophrenia and other major psychiatric disorders, the authors devised an outpatient multifamily treatment program specifically meant to treat difficult cases of adolescent anorexia nervosa following inpatient treatment. This article describes the therapeutic program, its goals and underlying theories, its structure and activities, its advantages and disadvantages. Our experience with this approach indicates that multifamily groups are a useful therapeutic alternative for cases that are not well suited for individual family therapy and remain fragile after inpatient treatment, thus being at risk for relapse and a chronic course.

Pero algunas familias (alrededor del 10%) responden mal a este enfoque, lo rechazan o lo encuentran insuficiente. La eficacia reconocida del enfoque multifamiliar (es decir un grupo constituido por los participantes de 5 a 7 familias) en el tratamiento de distintos desordenes psiquiátricos graves nos han animados a desarrollar un programa de terapía multifamiliar ambulatoria aplicado a algunos adolescentes anoréxicos y a sus familias después de una hospitalización. El artículo describe el proyecto terapeútico, sus teorías subyacentes y objetivos, su estructura y sus actividades, sus ventajas y sus dificultades. El grupo multifamiliar constituye una modalidad terapeútica interesante para acompañar los casos dificiles, permaneciendo en situación de fragilidad después de la hospitalización y presentando un riesgo importante de recaída y cronicisación.
