Summary, etc. Le case management est un processus qui évalue l’interaction des individus avec l’environnement. Il permet d’établir un plan global d’intervention suffisamment souple pour être réactif et intégré. Le case management vise à aider les sujets vulnérables à maintenir et améliorer leurs compétences dans leur environnement habituel. Ce processus de soins et de services est multidimensionnel, dynamique et longitudinal. Le Québec, les États-Unis et la Grande Bretagne développent des modèles de case management. Dans le contexte français, il existe des formes proches du case management dans les pratiques de secteurs qu’ils s’agissent des pratiques infirmières ou des travailleurs sociaux. On parle alors de la référence soignante ou éducative, des fonctions de coordonnateur des soins ou de parcours/projet. Cet article a pour objectif de présenter les formes et présupposés théoriques du case management, d’en discuter les conditions de mise en œuvre, les limites et l’adaptation possible de cette approche en France. Enfin, un lien sera fait entre le case management et l’intégration des soins et des services en réseau.

Summary, etc. Case management: what skills are needed to support comprehensive and coordinated mental health after care?Case management is a process assessing the correlation between the individuals and their environment. This process defines a global plan of services, flexible intervention, aimed at being reactive and integrated. Case management aims to helping vulnerable patients to support and improve their skills. This process of care and global services offers multidimensional, dynamic and long-time after care. Canada, the United States and Great Britain have already developed this kind of approach. In France, there are some similar forms of practices (nurses and social workers). This article aims to introduce the different forms of work and case management theory. Then, we will discuss the conditions of its implementation, theirs limits and the adaptation of this clinical and organizational approach in France. Finally, a link will be made between case management and the models of integrated services and network integrated services.
