| Summary, etc. | La douleur procédurale reste une douleur « indue », particulièrement dans les populations les plus fragiles comme les patients âgés cumulant douleurs multiples, comorbidités sévères et polymédication. Les opioïdes forts à libération immédiate se positionnent en première ligne de prescription anticipée dans ce contexte, de par leur maniabilité. Ils permettent de mieux contrôler les douleurs liées aux soins. Trois mots clés permettent une juste prescription antalgique : analyser, comprendre/anticiper et agir. Une bonne connaissance des opioïdes est un prérequis indispensable, afin de maîtriser leur maniement et ainsi améliorer la qualité de vie des sujets âgés vulnérables et souvent dépendants. Ils doivent rester sous surveillance constante le temps de leur utilisation. Cette approche algogériatrique se veut globale, où le positionnement du médicament opioïde est souvent incontournable. Contrôler les douleurs procédurales est un défi du quotidien en gériatrie et relève de la bientraitance. |

| Summary, etc. | Healthcare-induced pain remains “undue”, particularly in the most fragile populations such as elderly patients with multiple pains, severe comorbidities and polymedication. Strong immediate-release opioids are positioned as the first line of anticipated prescription in this context, due to their ease of use. They allow for better control of care-related pain; three key words allow for an accurate analgesic prescription: analyze, understand/anticipate, and act. A good knowledge of opioids is an essential prerequisite for mastering their handling and thus improving the quality of life of vulnerable and often dependent elderly patients. They must remain under constant supervision during their use. This geriatric pain management approach is intended to be comprehensive, where the positioning of the opioid medication is often essential. Controlling procedural pain is a daily challenge in geriatrics and is a matter of good treatment. |
