| Summary, etc. | La réalisation pratique d’un traitement par stimulation magnétique transcrânienne (rTMS) est aujourd’hui peu intégrée dans les habitudes de soin en France. De fait, si les connaissances théoriques et scientifiques sont maintenant répandues, les considérations pratiques peuvent demeurer obscures pour les psychiatres qui ne pratiquent pas eux-mêmes la rTMS. Cet article a pour objectif de répondre à certaines de ces questions du point de vue du médecin pratiquant en centre de neuromodulation. Nous aborderons la question du positionnement actuel de la rTMS en France, influencé à la fois par les contextes scientifique et réglementaire, et celle de sa place face à l’électroconvulsivothérapie. Nous nous demanderons comment certains facteurs, notamment cliniques et pharmacologiques, peuvent modifier la conduite à tenir. Nous verrons enfin quelles propositions peuvent être faites pour optimiser l’effet de la rTMS. |

| Summary, etc. | The practical implementation of a rTMS treatment is currently an atypical practice in France, particularly due to the absence of official recognition by the health authorities. Although the theoretical and scientific knowledge is now widespread, practical considerations may remain unfamiliar to psychiatrists who do not actually administer rTMS themselves. This article aims to answer some of these questions from the point of view of doctors practicing in a neuromodulation center. We discuss the current positioning of rTMS in France, influenced by both scientific data and the regulatory context, and its place in relation to ECT. We outline how certain factors, particularly clinical and pharmacological, can influence the practitioner’s attitude. Finally, we develop how a rTMS treatment is carried out and offer suggestions for optimizing its effect, in particular by integrating it into a global care strategy. |

| Summary, etc. | La implementación práctica del tratamiento con estimulación magnética transcraneal (EMTr) aún no está bien integrada en las prácticas sanitarias francesas. De hecho, si bien los conocimientos teóricos y científicos están ahora ampliamente difundidos, las consideraciones prácticas pueden seguir siendo oscuras para los psiquiatras que no practican ellos mismos la EMTr. Este artículo pretende responder a algunas de estas preguntas desde el punto de vista del médico que ejerce en un centro de neuromodulación. Abordaremos la cuestión del posicionamiento actual de la EMTr en Francia, influida a la vez por los contextos científico y normativo, y su lugar en relación con la terapia electroconvulsiva. Nos preguntaremos cómo ciertos factores, especialmente clínicos y farmacológicos, pueden modificar la actuación a seguir. Por último, veremos qué propuestas se pueden hacer para optimizar el efecto de la EMTr. |
