| Summary, etc. | Le syndrome cardio-rénal (SCR) chez le chien et le chat désigne l’interaction pathologique entre dysfonction cardiaque et rénale, où la défaillance d’un organe aggrave celle de l’autre. Ce syndrome, souvent sous-diagnostiqué, complexifie la prise en charge thérapeutique, car il nécessite de trouver un équilibre délicat entre maintien de la perfusion rénale, contrôle de la pression artérielle, prise en charge de la congestion et limitation de la progression de la maladie rénale chronique (MRC). Cet article propose une synthèse des recommandations actuelles (IRIS, ACVIM) et des études récentes concernant la gestion combinée de l’insuffisance cardiaque, de la MRC et de l’hypertension artérielle chez les animaux en syndrome cardio-rénal. L’approche thérapeutique repose sur une individualisation des traitements : choix raisonné des diurétiques, inhibiteurs du SRAA (IECA, antagonistes des récepteurs de l’angiotensine II), bloqueurs calciques et bêta-bloquants, tout en tenant compte du statut volémique, du risque d’azotémie et de l’évolution de la fonction rénale. La surveillance rapprochée et les ajustements thérapeutiques progressifs sont essentiels pour optimiser la qualité de vie de ces animaux difficiles à prendre en charge. |

| Summary, etc. | Cardiorenal syndrome (CRS) in dogs and cats refers to the pathological interplay between cardiac and renal dysfunction, where failure of one organ exacerbates the other. Frequently underdiagnosed, CRS presents significant therapeutic challenges, as clinicians must balance renal perfusion, blood pressure control, volume status, and the progression of chronic kidney disease (CKD). This article provides an overview of current recommendations (IRIS, ACVIM) and recent studies on the integrated management of heart failure, CKD, and systemic hypertension in cardiorenal patients. Therapeutic strategies must be tailored individually, taking into account the patient’s clinical profile and disease stage. This includes thoughtful use of diuretics, RAAS inhibitors (ACE inhibitors, angiotensin II receptor blockers), calcium channel blockers, and beta-blockers, while closely monitoring hydration status, azotemia risk, and renal function evolution. Frequent reassessment and gradual treatment adjustments are key to improving outcomes and quality of life in these complex cases. |
