| Summary, etc. | L’évaluation du thermalisme pose des problèmes méthodologiques spécifiques qui sont partagés par certaines autres thérapies “artisanales”.Chaque méthode a ses avantages et ses inconvénients et aucune ne peut réellement prétendre être supérieure aux autres.Nous observons que le design de référence jusqu’à présent était la comparaison à une population de patients sur liste d’attente, ce qui n’est pas complètement satisfaisant car il apparaît à l’usage qu’elle surestime la différence entre le groupe traité et le groupe témoin. La méthode de Zelen paraît plus fiable mais n’est pas facile à mettre en œuvre. Une randomisation selon les préférences, qui n’a encore jamais été utilisée, aurait peut-être plus de qualités mais entraîne des coûts de fonctionnement sensiblement plus élevés. La comparaison à un traitement validé est utilisable dans quelques cas mais il faut préférer une étude de supériorité sur une étude d’équivalence pour laquelle il persiste toujours un doute sur le manque de puissance statistique. |

| Summary, etc. | The evaluation of spa therapy poses specific methodological problems that are shared by certain other “artisanal” therapies.Each method has its advantages and disadvantages, and none can really claim to be superior to the others.We note that the reference design up to now has been a comparison with a population of patients on a waiting list, which is not entirely satisfactory, as it overestimates the difference between the treatment and control groups. Zelen’s method seems more reliable but is not easy to implement. Preference randomization has not been performed yet may be better, but entails significantly higher operating costs. Comparison with a validated treatment can be used in some cases, but a superiority study should be preferred to an equivalence study, for which there is always a doubt about the lack of statistical power. |
