| Summary, etc. | Les options thérapeutiques à disposition pour le traitement du glaucome chez le cheval sont médicales et chirurgicales. L’objectif du traitement est de diminuer la PIO pour apporter du confort au cheval et préserver la vision si elle est toujours présente. La diminution de la PIO s’effectue en régulant le flux d’humeur aqueuse, principalement en diminuant sa production et/ou en augmentant son élimination. Le traitement médical est facilement accessible, et peut être administré par voie locale ou systémique pour certaines molécules. Il comporte généralement des inhibiteurs de l’anhydrase carbonique et des β-bloquants, qui permettent la réduction de la production d’humeur aqueuse. Le traitement chirurgical comporte des techniques permettant de réduire la production d’humeur aqueuse et des techniques filtrantes. Il peut être divisé en deux grandes catégories selon le statut visuel du cheval. La cyclophotocoagulation au laser diode est, pour le moment, la technique la plus prometteuse pour la préservation de la vision. En cas de perte de vision et d’inconfort marqué du cheval, une destruction chimique du corps ciliaire ou une énucléation doivent être envisagées. La réponse au traitement est, dans la plupart des cas, rapide, mais le pronostic visuel au long terme reste sombre. La réalisation d’études comparatives sur l’efficacité des options thérapeutiques à disposition sur des chevaux glaucomateux est nécessaire, afin d’améliorer la prise en charge du glaucome chez le cheval. |

| Summary, etc. | Therapeutics options in the treatment of horse glaucoma are medical and surgical. The primary goal is to lower the IOP in order to provide comfort to the patient and preserve vision if it still remains. Lowering the IOP is done using two mechanisms : decreasing aqueous humour formation and/or increasing aqueous humour outflow. Medical treatment is easily available under prescription and can be administered topically or systematically for some molecules. Carbonic anhydrase inhibitors and beta-blockers are the most frequently used and work by decreasing aqueous humour production. Surgical treatment can be divided in two categories depending on the visual status of the patient. Currently, diode laser cycloablation is the most promising technique for vision preservation. In case of non-visual and painful glaucomatous eye, humane treatments like chemical ciliary body ablation or enucleation must be considered. Response to treatment is, in most cases, quick but the long-term visual prognosis is bad. Prospective comparative clinical trials on glaucomatous horse eyes are necessary to improve the treatment of horse glaucoma. |
