| Summary, etc. | Au travers du cas clinique d’un adolescent de 14 ans, Pierre, hospitalisé en pédopsychiatrie pour des troubles du comportement importants, nous illustrons dans cet article comment une posture thérapeutique d’ouverture face à l’incertitude peut être un outil institutionnel fécond, nourrissant la rencontre et les soins offerts aux patients. La tendance actuelle en pédopsychiatrie semble s’appuyer sur toujours plus de « certitudes » (diagnostiques, thérapeutiques, de causalités, neurobiologiques). Nous soutenons ici la validité d’une approche psychopathologique, thérapeutique et institutionnelle tolérant un certain degré d’incertitude, sans nier pour autant l’importance d’un repérage nosographique précis. La prise en charge de Pierre nous a en effet confrontés aux différentes facettes de l’incertitude : sur le plan clinique, pratique et relationnel. Ce n’est qu’en coconstruisant, avec lui, de nouvelles certitudes, plus souples et moins figées, qu’une alliance thérapeutique inespérée a pu se tisser. |

| Summary, etc. | Through the clinical case of a 14-year-old adolescent, Pierre, hospitalized in child psychiatry for severe behavioral disorders, we illustrate in this article how a therapeutic posture of openness in the face of uncertainty can be a fruitful institutional tool, nourishing the encounter and care offered to patients. The current trend in child psychiatry seems to rely increasingly on “certainties” (diagnostic, therapeutic, causal, neurobiological). We support here the validity of a psychopathological, therapeutic and institutional approach that tolerates a certain degree of uncertainty, without denying the importance of precise nosographic identification. Indeed, Pierre’s care has brought us face to face with the different facets of uncertainty: clinical, practical and relational. It was only by co-constructing new, more flexible and less rigid certainties with him that we were able to forge an unexpected therapeutic alliance. |

| Summary, etc. | À través del caso clínico de un adolescente de 14 años, Pierre, hospitalizado en psiquiatría infantil por trastornos graves del comportamiento, ilustramos en este artículo cómo una postura terapéutica de apertura frente a la incertidumbre puede constituir una herramienta institucional fértil, que enriquece el encuentro y los cuidados brindados a los pacientes. La tendencia actual en psiquiatría infantil parece basarse cada vez más en “certezas” (diagnósticas, terapéuticas, causales, neurobiológicas). Aquí defendemos la validez de un enfoque psicopatológico, terapéutico e institucional que tolere cierto grado de incertidumbre, sin por ello negar la importancia de una identificación nosográfica precisa. El abordaje del caso de Pierre nos enfrentó efectivamente a las diferentes facetas de la incertidumbre: en los planos clínico, práctico y relacional. Solo mediante la coconstrucción, junto a él, de nuevas certezas más flexibles y menos rígidas, pudo establecerse una alianza terapéutica inesperada. |
