Il repose sur une approche immersive et innovante grâce à la co-animation d’ateliers par une patiente-partenaire et une pharmacienne hospitalière. Ces temps d’échange permettent une meilleure compréhension de l’expérience patient, tout en consolidant les compétences relationnelles et l’interprofessionnalité entre acteurs du territoire. |
