| Summary, etc. | Although two clusters have been identified in France, constitutional factor XI deficiency is a rare disorder. Acquired factor XI deficiency is extremely rare. The management of factor XI deficiency is not staightforward because of the unpredictable bleeding tendency that does not clearly relate to the factor XI level. Other haemostastis parameters have to be taken into account to evaluate the bleeding tendency. We report the cases of a congenital factor XI deficiency, an acquired factor XI deficiency and a von Willebrand disease associated to a factor XI deficiency. On the other hand, some interferences can lead to underestimation of factor XI and we report the case of an interference by lupus anticoagulant. The objective of this review is to better understand how to manage a reduced factor XI level. |

| Summary, etc. | Le déficit constitutionnel en facteur XI est rare, bien que deux clusters aient été identifiés en France. Le déficit acquis en facteur XI est exceptionnel. Le management de ce déficit est compliqué par le manque de corrélation entre la symptomatologie hémorragique et le taux de facteur XI. Par ailleurs, d’autres paramètres de l’hémostase peuvent avoir un impact sur la symptomatologie hémorragique. Nous rapportons ainsi le cas d’un déficit congénital en facteur XI, d’un déficit acquis par anticorps antifacteur XI et le cas d’un déficit associé en facteur XI et en facteur Willebrand. D’autre part, des interférences analytiques peuvent entraîner une sous-estimation du facteur FXI, et nous rapportons le cas d’une interférence par anticoagulants circulants. L’objectif de cet article est de donner les éléments nécessaires à la conduite à tenir devant un facteur XI abaissé isolément ou non. |
