| Summary, etc. | Les auteurs ont tenté dans ce travail, d’apporter un éclairage socio-familial à une schizophrénie résistante. Il s’agit d’un mauritanien de 22ans, suivi en ambulatoire en Mauritanie. L’exacerbation des troubles a justifié son hospitalisation au Sénégal, par sa famille soucieuse d’échapper à la stigmatisation. Cette admission a duré deux mois sans rémission clinique après une intolérance aux neuroleptiques classiques de forme injectable. Les efforts d’adaptation du patient imposés par le contexte migratoire dans ce milieu nouveau, semblent avoir été compromis par une perception de la réalité déjà altérée par la maladie. Le retour à domicile et une réorganisation du schéma de fonctionnement familial ont engendré une sédation des symptômes en 15 jours. Sous cet angle, l’agitation persistante en cours d’hospitalisation a pu constituer une réponse partielle du patient à une distribution dysfonctionnelle des rôles parentaux. |
