| Summary, etc. | Guérir un trouble de l’usage d’un produit ou une maladie bio-psycho-sociale ressemble à un défi – voire à un paradoxe ! Mais l’importance des dommages liés à la nocivité du produit ou à la dépendance est un stimulant puissant pour les recherches. 40 à 50 ans d’addictions en France n’ont vu émerger que de rares molécules, souvent à effet partiel et décevantes. Les traitements de substitution (nicotine et opiacés) sont les seuls, à ce jour, qui font preuve avec constance d’une amélioration de la qualité de vie et d’une diminution des dommages. Nous avons essayé de mettre en visibilité toutes les molécules qui ont été ou sont encore disponibles dans ce champ, ainsi que des perspectives réalistes. Il faut insister sur l’importance des prises en charge globales – dont les médicaments ne représentant parfois qu’une petite part – pour accueillir, accompagner, améliorer la vie des usagers qui consultent les équipes spécialisées. |

| Summary, etc. | Curing a substance use disorder or a biopsychosocial illness seems like a challenge—even a paradox! But the extent of the damage caused by substances or by addiction is a powerful stimulus for research. Forty to fifty years of addiction research in France have yielded only a few medications, often with partial and disappointing effects. Substitution treatments (nicotine and opiates) are the only ones to date that consistently demonstrate an improvement in quality of life and a reduction in harm. We have attempted to highlight all the medications that have been or are still available in this field, as well as realistic prospects. It is crucial to emphasize the importance of comprehensive care—of which medication sometimes represents only a small part—in order to welcome, support, and improve the lives of users who consult specialist teams. |
