Summary, etc. IntroductionBurkholderia pseudomallei is a Gram-negative saprophytic bacillus that causes melioidosis. The infection is endemic in South-East of Asia and Northern Australia. B. pseudomallei has been designated as bioterrorism agent and its manipulation should be done in a biological safety level 3 capability. Workers in laboratories may be accidentally exposed to B. pseudomallei before its identification, with a risk of laboratory-acquired melioidosis.ObjectivesWe want to describe a case of melioidosis occurred in our hospital and its management at laboratory. The objective of this article is to provide guidance to microbiologists confronted with a suspicious case of B. pseudomallei on the management of the exposition.ConclusionsWe report here a couple of microbiological arguments that can usually guide microbiologists towards presumptive identification of B. pseudomallei. This case report shows the importance of MALDI-TOF MS accurate databases to ensure accurate microbial identification and antibiotic prophylaxis adapted to individuals who were exposed. We also want to underline the importance of developing an effective strategy of prevention against any accidental exposure that can occur in a microbiological laboratory.

Summary, etc. IntroductionBurkholderia pseudomallei est un bacille Gram négatif saprophyte de l’environnement responsable de la mélioïdose. L’infection est endémique dans le Sud-Est de l’Asie et le Nord de l’Australie. B. pseudomallei a été désignée comme agent de bioterrorisme et sa manipulation nécessite un laboratoire de sécurité microbiologique de niveau L3 mais tous les laboratoires n’en disposent pas. Le personnel de laboratoire peut donc être accidentellement exposé à B. pseudomallei avant l’identification bactérienne des cultures, avec un risque de mélioïdose acquise en laboratoire.ObjectifsNous souhaitons décrire un cas de mélioïdose survenu dans notre hôpital et sa gestion au laboratoire. L’objectif de cet article est de guider les microbiologistes confrontés à un cas suspect de B. pseudomallei sur la gestion de l’exposition au sein du laboratoire.ConclusionsNous rapportons ici les arguments microbiologiques ­pouvant orienter les microbiologistes vers une identification présomptive de B. pseudomallei. Cet article montre l’importance de disposer de bases de ­données de MALDI-TOF MS solides pour assurer une identification microbienne précise et ainsi guider le choix d’une antibioprophylaxie adaptée aux individus exposés. Nous souhaitons également souligner ­l’importance de développer une stratégie efficace de prévention contre toute exposition accidentelle pouvant survenir dans un laboratoire de microbiologie.
