| Summary, etc. | Cet article explore l’usage des neuroleptiques à action prolongée (NAP) dans la psychiatrie française de 1966 à aujourd’hui, en articulant trois ambitions complémentaires. D’un point de vue historique, il retrace les dynamiques d’appropriation des NAP, montrant comment leur diffusion accompagne, sans les déterminer, les transformations institutionnelles de la psychiatrie, notamment à travers la sectorisation. Sur le plan socio-anthropologique, l’analyse des pratiques, basée sur des données ethnographiques, des entretiens et des archives médicales, révèle des usages variés des NAP, entre négociation thérapeutique, gestion des patients « observants » et redéfinition des lieux du soin en dehors de l’hôpital. Enfin, l’ambition théorique de l’article est d’interroger la relation complexe entre innovation pharmacologique et transformations institutionnelles. Loin d’un déterminisme technologique, les NAP apparaissent comme des « traceurs » des mutations psychiatriques, dont les significations évoluent selon les contextes. |

| Summary, etc. | This article explores the use of long-acting antipsychotics (LANAs) in French psychiatry from 1966 to the present day, articulating three complementary ambitions. From a historical point of view, it traces the dynamics of LANA appropriation, showing how their diffusion accompanies, without determining, the institutional transformations of psychiatry, notably through sectorization. On a socio-anthropological level, the analysis of practices, based on ethnographic data, interviews and medical archives, reveals the varied uses of LANAs, between therapeutic negotiation, management of “compliant” patients and redefinition of care settings outside the hospital. Finally, the article’s theoretical ambition is to examine the complex relationship between pharmacological innovation and institutional transformations. Far from being technologically determined, LANAs appear to be “tracers” of psychiatric change, whose meanings evolve according to context. |
