Summary, etc. Cet article s’appuie sur la théorie philosophique des valeurs pour interpréter l’expérience vécue des inégalités raciales en matière de santé mentale, ouvrant ainsi la voie à la co-construction comme base de meilleurs soins. L’article se concentre à titre d’exemple sur l’usage disproportionné de la contrainte en psychiatrie chez les jeunes hommes Noirs par rapport à leurs pairs Blancs. La section 1 présente un compte rendu à la première personne de l’impact des inégalités raciales en santé mentale. La section 2 s’appuie sur des sources africaines et européennes de la philosophie des valeurs pour interpréter les expériences décrites dans la section 1. Les insights-clés sont que le monde vécu des valeurs ne se situe pas à l’intérieur des individus mais dans l’espace inter-personnel, et que c’est dans cet espace que les valeurs implicites de la blanchité entraînent des inégalités raciales en matière de santé mentale. Comprise en ces termes, l’usage disproportionné de la contrainte en psychiatrie chez les jeunes hommes Noirs reflète des conflits de valeurs non reconnus survenant dans l’espace intersubjectif entre, dans ce cas, les patients Noirs et les psychiatres Blancs (ou formés par des Blancs). La section 3 explore l’impact pratique potentiel de ces insights pour la co-construction basée sur l’égalité des voix pour l’égalité raciale en santé mentale. Nous évoquons brièvement les implications plus larges de nos conclusions pour la santé mentale et pour la médecine centrée sur la personne dans son ensemble.

Summary, etc. This article draws on philosophical value theory to interpret the lived experience of racial inequalities in mental health, thereby pointing the way to co-production as the basis of improved clinical care. The paper focusses by way of example on the disproportionate use of psychiatric compulsion in young black men compared with their white counterparts. Section 1 provides a first-person account of the impact of racial inequalities in mental health. Section 2 draws on Afrocentric and Eurocentric sources in the philosophy of values to interpret the experiences described in section 1. The key insights are that the lived world of values is located not within individuals but in inter-personal space; and that this is the space within which implicit values of whiteness drive racial inequalities in mental health. Understood in these terms, the disproportionate use of psychiatric compulsion in young black men reflects unacknowledged conflicts of values arising in the inter-subjective space between, in this instance, black patients and white (or white trained) psychiatrists. Section 3 explores the potential practical impact of these insights for co-production based on equality of voices for racial equality in mental health. We point briefly to the wider implications of our conclusions for mental health and for person-centered medicine as a whole.
