| Summary, etc. | Les virus influenza de type A qui circulent chez les oiseaux sont responsables de cas sporadiques de grippe zoonotique. En cas d’adaptation à cet hôte, ils peuvent être à l’origine d’une pandémie grippale, et les pandémies qui ont marqué l’histoire de la grippe sont toutes d’origine zoonotique. Depuis 2022, le monde fait face à une panzootie aviaire due au virus influenza A(H5N1) de clade 2.3.4.4b qui a touché les oiseaux d’élevage. Ce virus, du fait de réassortiments multiples, s’est adapté à de nombreux mammifères, dont certains sont en interface avec l’Homme. Il existe un risque non nul qu’il s’adapte à l’Homme et entraîne une nouvelle pandémie grippale. De nombreux pays ont révisé en conséquence leurs politiques de prévention vis-à-vis de ce risque potentiel pour la santé humaine. Cette prévention repose sur des mesures de biosécurité, une surveillance virologique renforcée, un dépistage et une prise en charge précoce des cas de grippes zoonotiques. La vaccination est essentielle. La vaccination annuelle contre la grippe saisonnière chez les professionnels exposés aux virus influenza aviaires limite le risque de réassortiment entre un virus saisonnier et un virus aviaire. Des vaccins adaptés au virus H5N1 enzootique actuel sont autorisés chez l’Homme et leur place, dans la période actuelle dite prépandémique, a été définie par la Haute Autorité de Santé. En cas de pandémie, ils pourraient avoir un rôle en attendant de disposer d’un vaccin adapté. |

| Summary, etc. | Influenza A viruses circulating in birds are responsible for sporadic cases of zoonotic flu. If they adapt to this host, they can cause a pandemic flu, and the pandemics that have marked the history of influenza are all of zoonotic origin. Since 2022, the world has been facing an avian panzootic due to the influenza A(H5N1) virus of clade 2.3.4.4b, which has affected farmed birds. Due to multiple reassortments, this virus has adapted to many mammals, some of which interact with humans. There is a non-zero risk that it will adapt to humans and cause a new influenza pandemic. Many countries have therefore revised their prevention policies in response to this potential risk to human health. This prevention is based on biosecurity measures, enhanced virological surveillance, screening and early management of zoonotic influenza cases. Vaccination is essential. Annual seasonal influenza vaccination for professionals exposed to avian influenza viruses limits the risk of reassortment between a seasonal virus and an avian virus. Vaccines adapted to the current enzootic H5N1 virus are authorised for use in humans, and their role in the current pre-pandemic period has been defined by the French High Authority for Health. In the event of a pandemic, they could play a role until a suitable vaccine becomes available. |
