| Summary, etc. | Dans la mesure où le ribociclib n’a pas de remboursement en France en situation adjuvante, le choix d’un traitement par inhibiteur de CDK4/6 repose uniquement sur la classification TNM et les caractéristiques clinico-pathologiques sur la base des résultats de l’étude MonarchE. Si demain, le ribociclib obtient un remboursement en France en situation adjuvante sur la base des résultats de l’étude NATALEE, il pourrait, en théorie, être prescrit dans cette situation pour les patientes ayant un cancer du sein de stade IIA N0 avec signature multigénique définissant un haut risque. Toutefois, pour ce sous-groupe, le bénéfice reste incertain (pas de données en survie globale). L’une des questions encore non résolues pour ces patientes ayant un cancer du sein RH+/HER2– « à risque intermédiaire » de rechute est bien celle de l’omission d’une chimiothérapie adjuvante, à présent que l’hormonothérapie peut être potentialisée par un inhibiteur de CDK4/6. Le concept de « réponse moléculaire » (conversion d’un sous-type agressif vers un sous-type plus indolent) ou bien celui de « réponse endocrinienne » (diminution dynamique du Ki67 après deux à trois semaines d’hormonothérapie), combiné à une signature multigénique, pourrait à l’avenir nous permettre d’identifier ces patientes. |

| Summary, etc. | Insofar as ribociclib is not refunded in France for the adjuvant treatment of breast cancers, the choice of treatment by CDK 4/6 inhibitors can be made only on the TNM classification and clinical-pathological characteristics on the base of results of the MonarchE study. If ribociclib were recognised as refundable as an adjuvant treatment on the basis of the results of the NATALEE study, it could, in theory, be prescribed to women with stage IIA N0 breast cancer with a multigenetic signature defined as high risk. However, for this sub-group, the benefits remain uncertain. The key question which remains unresolved for patients with an “intermediate” risk of a relapsing HR+/HER2- breast cancer is whether to omit adjuvant chemotherapy now that hormone therapy can be potentiated by a CDK 4/6 inhibitor. The concept of a “molecular response” converting an aggressive sub-type to a more indolent sub-type or that of an “endocrine response”, dynamically reducing Ki67 after two to three weeks of hormone therapy, combined with a multigene signature, could, in the future, enable these patients to be identified. |
