| Summary, etc. | Objectif : Le projet de réorganisation des services hospitaliers de l’Établissement hospitalier universitaire d’Oran (EHUO) en pôles d’activité est un exercice complexe et un challenge qui fait face à des obstacles structurels, organisationnels et surtout culturels. Les relations entre l’EHUO et la tutelle centrale ne laissent aucune latitude en termes d’autonomie. Le financement continue d’être assuré par le biais des subventions. De plus, aucun accompagnement réglementaire et technique n’est offert aux acteurs de l’établissement pour la construction du projet local et encore moins pour toute restructuration. Notre objectif était de décrire et d’analyser l’organisation projetée sous la forme de pôles d’activités. Méthodes : Nous nous sommes basés sur les rares documents disponibles produits par l’administration (notes et instructions) et sur le document d’analyse des projets de services. Résultats : Les pôles sont proposés à partir des esquisses des projets de services pris l’un après l’autre, sans qu’il y ait une reformulation dans le cadre d’une vision commune, ni une mise en cohérence du point de vue des interactions induites. Les effets théoriquement attendus, comme l’autonomie, la délégation de gestion des pôles, ne sont pas à l’ordre du jour. Le contrat interne ne semble pas être au centre des débats. Les économies d’échelle et la mutualisation des ressources sont abordées en filigrane. Conclusion : D’importants préalables sont nécessaires pour porter cet ambitieux projet, très structurant par ailleurs. Les acteurs de l’EHUO gagneraient à s’entendre sur une vision commune et des valeurs à défendre ensemble dans le cadre d’un projet construit collectivement pour une bonne gouvernance. |

| Summary, etc. | Analysis of activity pole restructuring at Oran University Hospital, Algeria Aim: The EHUO project of reorganization of medical units into the new activity poles is a complex exercise and a challenge in relation to structural, organizational, and especially cultural barriers. The relationships between the EHUO and the Ministry of Health leave no room for autonomy. Funding continues to be provided through grants. Furthermore, no regulatory and technical support is available to the EHUO actors for development of the local project and even less for restructuring. Methods: The authors describe and analyze the planned new organization from limited material available produced by the administration (notes and instructions) and the services projects analysis document prepared by our team. Results: The proposed poles are based on draft projects integrating the various units, with no reformulation in the context of a shared vision or a coherent point of view including the interactions between all parts of the project. Theoretically expected effects, such as autonomy, and management contracts for instance, are not on the agenda. The internal contract does not appear to be at the heart of the debate. Economies of scale and sharing of resources are only vaguely discussed. Conclusion: Important prerequisites are necessary for this ambitious and highly structuring project. The EHUO actors must strive to reach an agreement on a common vision and values to be protected together as part of a collectively designed project to ensure good governance. |
