| Summary, etc. | ObjectiveFor the training of junior doctors (DJ) in medical regulation, we use a card game to determine by lot the scenarios for the simulation sessions. The aim of this study was to check that the situations proposed were in line with the reality of calls handled at SAMU 93.MethodsThe card set evaluated consisted of 53 cards in four sets corresponding to (1) the caller’s mode of communication (7 cards, referring to Snow White’s Seven Dwarfs), (2) the relationship between caller and patient (14 cards), (3) the patient’s age (11 cards) and (4) the reason for call (21 cards). The investigators were recently trained DJs (N=7) and a supervisor. In successive calls, they recorded the patient’s age and gender, the caller’s relationship with the patient, his or her mode of communication and the reason for referral (multiple responses possible). The objective was to include 250 calls.Results328 consecutive calls included by six investigators: 150 (46%) men and 178 (54%) women with a median age of 41 (23-68) years. Callers in descending order: themselves (28%), a carer (16%) and a teacher (11%). Five cards were added. Communication mode in descending order: “normal” (45%), “simpleton” (16%) and “shy” (12%). Reasons for referral: 70, including abdominal pain (11%), chest pain (10%) and fall from height (10%). Trauma accounted for 75 (22%) calls. Twelve cards were added.ConclusionThis study has enabled to validate a realistic card game in line with the activity of SAMU 93 to determine the scenarios for simulation sessions, with 51,205 possible combinations. This game could be generalized and adapted to local specificities. |

Cette étude avait pour objectif de vérifier que les situations proposées étaient conformes à la réalité des appels gérés au SAMU 93.MéthodesLe jeu de carte évalué comportait 53 cartes en quatre lots correspondant (1) au mode de communication de l’appelant (7 cartes, renvoyant aux sept nains de blanche-neige), (2) au lien entre l’appelant et le patient (14 cartes), (3) à l’âge du patient (11 cartes) et (4) au motif de recours (21 cartes). Les investigateurs étaient les DJ récemment formés (N=7) et un encadrant. Ils ont, lors d’appels successifs, relevé l’âge et le genre du patient, le lien de l’appelant avec le patient, son mode de communication et le motif de recours (plusieurs réponses possibles). L’objectif était d’étudier 250 appels.Résultats328 appels consécutifs inclus par six investigateurs : 150 (46 %) hommes et 178 (54 %) femmes d’âge médian : 41 (23-68) ans. Appelant par ordre décroissant : lui-même (28 %), un soignant (16 %) et un enseignant (11 %). Cinq cartes ont été ajoutées. Mode de communication par ordre décroissant : « normal » (45 %), « simplet » (16 %) et « timide » (12 %). Motifs de recours : 70 ont été relevés, dont : douleurs abdominales (11 %), douleurs thoraciques (10 %) et chute de sa hauteur (10 %). La traumatologie représentait 73 appels (22 %). Douze cartes ont été ajoutées et trois ont été modifiées. Soit un nouveau jeu de 75 cartes.ConclusionCette étude a permis de valider un jeu de carte réaliste et conforme à l’activité du SAMU 93 pour déterminer les scenarios des séances de simulation avec 51 205 combinaisons possibles. Ce jeu pourrait être généralisé et adapté aux spécificités locales. |
